With your permission Information about your Therapy is shared with relevant team members and third parties such as, but not limited to, solicitors, case managers, Physiotherapists and Occupational Therapists, SALT etc. The information is shared to enhance and promote your rehabilitation ensuring a holistic approach to your rehabilitation is achieved.

    2. Information will only be shared with the parties in 4.1 once a consent form has been signed.
